Provide a brief biography
Provide a comprehensive supportive essay explaining why the nominee deserves the honor
If selected, you will be asked to provide at least 3 photographs. You can provide either digital images by email or actual photographs (if you would like them returned, please provide a self addressed, stamped envelope).
If selected, be prepared to have the nominee fill out an image release form - although this steals the element of surprise, it is required by law - spoil sports!
